A decade of evidence shows that the androgen receptor CAG repeat polymorphism conditions the peripheral effects of testosterone across many tissues. Longer repeat tracts blunt receptor transactivation and thereby influence male sexual function and fertility, body composition, bone metabolism, cardiovascular risk, cancer risk, psychiatric status, and neurodegenerative vulnerability. This review surveys the literature and assigns the polymorphism a central role in modulating systemic androgen action. It provides the broad physiological map against which AR-signaling disruptions in post-drug syndromes can be interpreted.
